Dear Mr. Jacob,

I am writing to inform you of my gestation and my aim to take the granted 12 weeks of motherhood leave available to me. I planned to start my motherhood leave on October 21 and return to the office on January 23, expecting any unpredictable things with my gestation and delivery.

I planned to make every aim to finish preference projects before my motherhood leave starts. I should be able to check in periodically from home. I will do my best to make up for any important project or customer developments I missed when I return to work. I planned to representative some of my duties to the Assistant Supervisor who works with me in my department during my motherhood leave for your added convenience.

If my plans change for any reason, I will let you know as soon as workable. Thank you in proceed for allowing me time away from the office so that I may produce for the birth, bond with my child and adapt to life as a new mother. The required motherhood leave form is attached along with a letter from my doctor confirming my gestation. Please contact me if you need any extra information or explanation.
