This is to inform you that despite repeated verbal warnings; the last one given by me personally, you have not shown any improvement and continue to come late for work, many a times in a drunken state. Moreover your team-mates and subordinates have complained that you have not discontinued using abusive language while at work.
Apart from a steep fall in your performance, such unethical behavior from a middle-level management employee is unacceptable.
This is the last warning for your callous attitude towards work and behavior unbecoming of an employee in our reputed organization.
Any recurrence will result in your dismissal from your post of Assistant Manager-Sales.
Since you have been an asset to our organization in the past, I have taken personal interest and requested the management to give you a last chance .
